Hi Serinvg,
Most Practice Management Software products employ 8-30 developers. It takes a lot of time to develop a new medical billing software and with all of the constant claim changes your staff needs to be ready for any kind of change. In reality I would assume it's cheaper to pay $100 - $400.00 a month per provider over having to create a software product. Or possibly look for a smaller software product already created and buy them out.
To view what Medical Billing Software Products are out their I recommend researching this website

If you do plan on creating your own software I would suggest researching clearinghouses that can support your medical office needs. The best PM billing software are the ones integrated with clearinghouses for ease of front office and billing office use. You can research this website for a list of clearinghouses:

Your medical billing software is only as good as your clearinghouse. In fact, with all of the new industry changes it truly is the clearinghouse that is making doctors the money faster with patient eligibility, claim scrubbing, cleaner claim submssion, easier ways to identify denied claims, Electronic Remittance Advice, Lock Box functionalities, eStatements. Billing services are more of a place to store the charge and appointment history.
